Role summary
This is an exciting opportunity for a Senior Community Nurse to join a friendly and supportive team. You will provide expert nursing care to patients in their homes, supporting those with complex health needs, long-term conditions, frailty, and end-of-life care. The role involves comprehensive assessments, developing person-centred care plans, and coordinating care to help patients remain safely at home. You will also provide clinical leadership and support the development of newly qualified nurses.
Key responsibilities
- Provide expert nursing care to patients in their own homes.
- Undertake comprehensive assessments and develop person-centred care plans.
- Coordinate care to help patients remain safely at home and promote independence.
- Contribute to the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics.
- Facilitate/support newly Qualified Nurses in their Preceptorship year.
- Assess, prescribe and deliver care packages for patients with long term conditions, palliative care and rehabilitation needs.
Essential criteria
- 1st level degree
- RN qualification
- Assessor / mentorship preparation; ENB 998 or equivalent
- Current NMC registration
- Evidence of continuous professional development
- Evidence of working at band 5 level within the community setting
- To facilitate / develop Presceptorship Nurses
- Knowledge of additional specialist areas through in-house training & short courses
- Good communicator
- Highly motivated
- Able to provide high standards of care
- Good observational & reporting skills
- Ability to manage stressful situations
- Concentrate when undertaking patient care & inputting data/patient records
- Good interpersonal skills
- A team player
- Work flexibly to accommodate patient/service needs
- Will be able to make own travel arrangements to patients' homes, clinics, base and meetings etc. as required.
- Be able to kneel, bend & stoop, and work in cramped environments
- Be able to manoeuvre limbs of around 5-6 kg
- Be able to manoeuvre patients using handling aids
- Have basic IT/standard keyboard skills

- The post holder be required to work shifts, on call, flexible hours.

Job summary
An exciting opportunity has arisen for a Senior Community Nurse to join our friendly and supportive Community Nursing Team. Working autonomously and as part of a multidisciplinary team, you will undertake comprehensive assessments, develop and evaluate person-centred care plans, and coordinate care to help patients remain safely at home. You will play a key role in admission avoidance, helping to facilitate timely hospital discharge, and promoting independence and self-management.
As a senior member of the team, you will provide clinical leadership, support service development, and contribute to the supervision and development of newly qualified nurses through structured preceptorship and mentorship.
