Key responsibilities
- Lead and coordinate PSIRF-aligned patient safety investigations across the Division.
- Apply systems-based methodologies such as SEIPS, Human Factors, and Just Culture.
- Conduct interviews, evidence reviews, timelines, and systems analysis.
- Produce high-quality investigation reports with clear findings and proportionate recommendations.
- Ensure compassionate, transparent engagement with patients, families, and carers.
- Support staff involved in incidents and promoting psychological safety.

- Evidence of training in Quality Improvement Methodology or equivalent experience gained working on improvement projects
- Understanding of the Quality Governance Agenda
- Understanding of the regulatory framework under the Health and Social Care Act 2008 and fundamental standards 2014.
Desirable criteria
- A Clinical Qualification/registered healthcare professional -desirable.
- Project management or equivalent -desirable A Masters level award or willingness to study master's level- desirable
- Knowledge of MH services and MH Law - desirable

We are seeking an experienced and motivated Patient Safety Incident Investigator to join our Goodall Division. This is an exciting opportunity to play a pivotal role in improving patient safety, driving learning, and shaping high-quality care across a diverse range of services.
You will lead systems-based, high-quality investigations under the Patient Safety Incident Response Framework (PSIRF), ensuring that learning--not blame--drives improvement.
