Role summary
Join Hampshire and Isle of Wight NHS Healthcare NHS Foundation Trust as a Community Healthcare Support Worker. You will deliver high-quality, person-centred care to patients in their own homes, working as part of a multi-disciplinary team. The role involves contributing to care planning and evaluation, operating within a 7-day a week service. A full driving license and access to a vehicle are required.
Key responsibilities
- Work under supervision following written care plans and care pathways devised by the Registered Nurse.
- Follow prescribed care plans and treatment protocols.
- Follow HPCT Policies and Procedures.
- Carry out prescribed tasks and report any variances to the Registered Nurse.
- Undertake delegated responsibility for Patient Support Teams administrative tasks.
- Provide general administration support to the team.
Essential criteria
- NVQ in Health care or equivalent or willingness to commit to training.
- Full UK Driving Licence.
- NVQ in Health care or equivalent or willingness to commit to training.
