Role summary
This is an exciting part-time opportunity for an experienced Occupational Therapist to join the Community Therapy Teams across Plymouth, primarily based in the North of the city. The role involves delivering holistic, person-centred, multidisciplinary care to support people in maintaining independence at home. Key responsibilities include supporting safe hospital discharge, maximising independence, preventing avoidable admissions, and contributing to referral triage. The service is forward-thinking and welcomes new ideas, offering support for professional development.
Essential criteria
- Working in a multidisciplinary team (MDT) with the ability to work flexibly.
- Extensive clinical experience in Health and/or Social Care OT intervention planning.
- Advanced post registration experience in a Health and social care setting.
- Documented evidence of Continued Professional Development (CPD).
- Multi-pathology assessment.
- Home and community assessments.
- Effective caseload management and discharge planning.
- Management, leadership and supervisory skills.
Desirable criteria
- Experience of working with staff from other agencies.
- Experience of supervising junior staff and/or assistants.
- Experience of orthotics and splinting, including fabrication of thermoplastic splints.
- Risk assessment in a variety of settings.

- Working knowledge of Health and Social Care legislation and its use in current practice.
