Role summary
This role is for a Housing Support Lead to provide expert oversight, coordination, and decision-making for housing pathways within the service. Key responsibilities
- Act as the specialist lead for housing within the service.
- Provide expert oversight, co-ordination and decision-making in relation to housing pathways, complex cases and discharge planning.
- Ensure housing is embedded into clinical pathways from admission through to discharge.
- Manage a small caseload of the Trust's most complex housing-related discharge cases.
- Drive service improvement by embedding housing into discharge pathways and monitoring housing-related delays.
- Provide strategic oversight to reduce unnecessary length of stay and improve patient flow.
Essential criteria
- Educated to degree level or equivalent experience in housing or related field
- Evidence of Continued Professional Development
- Significant experience of and demonstrable achievement in housing and homelessness practice
- Experience of partnership working within and across statutory and non-statutory teams and agencies
- Experience of leading service improvements or developing operational processes
- Knowledge and understanding of relevant areas of housing policy and practice including the law relating to local housing authority homelessness duties and the allocation of social housing

Job summary
