Role summary
This role is for a skilled and motivated Assistant Practitioner to join the Integrated Reablement Team in Nottingham City. You will work within a multidisciplinary team to support patients in their homes, aiming to maximise independence through rehabilitation and crisis intervention. The service prevents unnecessary hospital admissions and facilitates timely discharges. You will plan, deliver, and evaluate person-centred rehabilitation programmes for adults with physical and mental health needs.
Key responsibilities
- Undertake individual treatment and interventions with clients under supervision of qualified staff.
- Accept responsibility for designated caseload, working independently to promote independence and meet service user needs.
- Assess, provide, fit, and demonstrate use of a wide range of equipment to promote independence.
- Assess for and commission a range of adaptations in line with multidisciplinary team guidelines.
- Maintain comprehensive and accurate records in line with employer policies.
- Undertake basic wound care tasks as delegated by the District Nurse and a range of nursing competencies as per the AP competency framework.
Essential criteria
- Educated to Foundation degree or equivalent in a health-related discipline or willingness to complete the Assistance Practitioner foundation degree.
- Experience of multi-disciplinary team working in a Health Care, Social Care, Voluntary Care setting.
- Experience managing own workload in a team setting
- Experience of working with individuals and/or groups within a Health Care, Social Care, Voluntary Care setting.
- Experience of working with individuals who have long-term conditions.
- Experience of record keeping and accurate data recording.
- Experience of working in the community and acute sector
- Experience of the UK Healthcare system
- Knowledge of differing professional area procedures and practices (e.g. nursing, physiotherapy or occupational therapy)
- Knowledge of Health and Social Care provision.
- Knowledge of need and risk in relation to Health Care provision in the community.
- Understanding the concepts of assessment and care planning.
- Knowledge of the roles of the members of the multi professional team.
- Awareness of promoting/enabling independence and self-care.
- Awareness of a range of activity of daily living equipment and adaptations.
- Effective communication skills (Verbal and written) and good interpersonal skills and IT skills
- Ability to prioritise and plan workload.

Desirable criteria

Job summary
An
exciting opportunity has arisen for a skilled, enthusiastic and motivated Assistant
Practitioner to join the Integrated Reablement Team, incorporating the Urgent
Community Response (UCR) Service, Pathway 1 Discharge to Assess (D2A) and
Reablement Services across Nottingham City.
Working
within a dynamic multidisciplinary team of Advanced Clinical Practitioners,
Physiotherapists, Occupational Therapists, Nurses, Pharmacy Technicians,
Assistant Practitioners, Rehabilitation Support Workers and Administrative
Staff, you will support patients to maximise independence through rehabilitation
and crisis intervention within their own homes. The service aims to prevent
unnecessary hospital admissions and facilitate timely, safe hospital
discharges.
As
part of the UCR service, you will support clinicians in responding to urgent
referrals from emergency and out-of-hours services, providing a rapid two-hour
response to crises such as falls, functional decline and acute illness.
Comprehensive in-house training will be provided, including falls recovery and safe
assistance from the floor.
The successful
candidate will plan, deliver and evaluate person-centred, goal-focused
rehabilitation programmes to adults with physical and mental health needs.
