Role summary
This is an exciting opportunity for a motivated and passionate individual to join the Wigan Home-Based Treatment Team as a Team Manager. The role involves overall responsibility for the day-to-day management, leadership, and development of the service. The service provides recovery-focused care to prevent hospital admissions and enable individuals to manage their illness at home. The Team Manager will also collaborate with the wider leadership team on service development initiatives.
Essential criteria
- RMN and evidence of significant professional development OR CSS/CQSW/Dip SW or equivalent Social Worker training and experience OR Occupational Therapist

- Experience of working in a supervisory capacity

- Knowledge & Skills
- A knowledge of the Home-Based Treatment Service Model of intervention and its effectiveness
- Sound knowledge of the factors involved in the development of poor mental health and key features of relapse prevention
- Ability to organise, prioritise and monitor own and others' workload
- Ability to implement and maintain good standards of professional practice to ensure a quality and efficient service
- The ability to promote good staff care by creating a supportive environment, which contains workload management, supervision, staff development and training opportunities

- A willingness to be flexible, proactive and to undertake duties which may fall outside traditional professional boundaries
- Post holder will be required to participate in an on-call system

Job summary
A rare and exciting opportunity has arisen for a motivated and passionate individual to join a forward thinking, dynamic Wigan Home-Based Treatment Team (HBTT) as Team Manager.
The post holder will take overall responsibility for the day-to-day management, leadership and development of the service. Using a team approach, the service will provide recovery-focused care with a view to preventing hospital admission and enabling individuals to manage their illness in their own home, thereby improving their quality of life.
The Team Manager will also work with the wider leadership team around all issues related to service development.
Main duties of the job
Retain overall responsibility for the management of the Home-Based Treatment Team.
Manage the workload of the Home-Based Treatment Team, ensuring that appropriate systems are in place for:
Allocation, transfer and discharge/closure of case responsibility, ensuring that transition in and out of the team is safely managed, in-line with Core Fidelity benchmarks.
Supporting patient flow both in and out of hospital, including support with gatekeeping and facilitating safe supported discharge from inpatient wards with HBT support.
