Role summary
This is an exciting opportunity for a Band 5 Bone Health Nurse to join Nottingham CityCare Partnership's Bone Health Service on a 12-month fixed term contract or secondment. The role is community-based, delivering evidence-based assessment, treatment, and follow-up for individuals with osteoporosis and those at risk of fragility fractures. You will undertake venepuncture, cannulation, and administer injection and IV medication in patients' homes. The position offers full-time and part-time options (22.5 hours per week for part-time) within a supportive multidisciplinary team.
Key responsibilities
- Deliver evidence based osteoporosis assessment, treatment, and fracture prevention care within a community setting.
- Undertake agreed clinical procedures including venepuncture, injections, cannulation, and administration of IV medication.
- Provide patient centred education and advice to promote self management, treatment adherence, and fracture risk reduction.
- Maintain accurate, timely clinical documentation in line with professional, legal, and organisational requirements.
- Work effectively as part of the multidisciplinary team, liaising with colleagues across community, primary, and secondary care.
- Participate in service delivery, audit, and quality improvement activities within the Bone Health Service.
Essential criteria
- Experience of delivering nursing care to a group of patients.
- Supervision of staff and students.
- Able to demonstrate an awareness of current national/local initiatives and guidelines in relation to osteoporosis
- A full, current UK driving license and access to a vehicle is required for this post. The vehicle must be roadworthy and insured for business use, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
- Ability to be flexible to meet the needs of the service
- Current RGN (Adult) registration with the Nursing and Midwifery Council (NMC)
- Willingness to complete IV therapy training within 3 months of taking up post, if not already trained and competent
- Demonstrate ability to prioritise, assess, plan, deliver and evaluate patient care
- Able to work independently but recognise own limitations
- Good communication skills- written and verbal
- Understanding of equality and diversity.
- Good time management and prioritisation skills. Able to work under pressure
- Ability to work well in a team
- Good IT skills

We have a full time position and a part time position available (working 22.5 hours per week)
This is a community-based role within a proactive, forward-thinking service delivering evidence-based assessment, treatment, and follow-up for individuals with osteoporosis and those at risk of fragility fractures. The successful applicants will join a supportive multidisciplinary team and contribute to the delivery of high-quality, person-centred care.
The role involves venepuncture, cannulation, and the administration of injection and IV medication in patients own homes.
Working pattern:
Service hours are Monday to Friday, 08:00 - 16:00.
The team is based at Bulwell Riverside, with the service covering Nottingham City and South Nottinghamshire County. The team offers a supportive environment with opportunities to learn, develop, and grow within community nursing.
