Role summary

Essential criteria

Job summary
Are you an experienced Occupational Therapist or Physiotherapist looking for an opportunity to utilise and develop your clinical skills & leadership abilities? If so, our Specialist Therapist position may be perfect for you.
The successful candidate will work within the Frailty team alongside other Occupational therapists and Physiotherapists. The team also includes a Consultant Geriatrician and specialist ACP's. The team covers 'front door services' to include: Frailty Assessment Unit, the Emergency Department and Acute Floor (AMU, AAU, SDEC and short stay ward). These clinical areas are fast paced with a high turnover of patients, and would therefore require a dynamic and holistic individual.
Our aim is to assess patients identified as frail on attendance or during admission, and use robust admission avoidance pathways to allow us to manage prompt discharge from the acute setting, inline with patient centred care and a home first model.
The successful applicant will also support the weekend discharge/patient progression rota.
