Role summary

Essential criteria
- Registered Nurses Learning Disabilities (RNLD)
- Post registration training / willingness to undertake training plus experience to postgraduate diploma level.

- Clinical Supervision / Mentorship qualification.
- Demonstrate knowledge and experience in supporting individuals who have a learning disability
- Experienced in delivering advice and support to a wide range of people.
- Experienced in partnership working.
- Experienced in managing risk.
- Experienced in delivering training.
- Previous community nursing experience desirable.
- Experience in supporting individuals with complex health needs
- Established knowledge of the health inequalities agenda for people with learning disabilities
- Understanding of Person Centred Planning

Job summary
To work within the Learning Disability Directorate as part of a multi-disciplinary integratedteam.
The post holder will provide specialist health and behaviour assessments to individuals referred to the learning disability team.
The post holder will be required to undertake positive behaviour support training if they do not posses this qualification.
Interventions will be delivered across different settings including the person's home.
There will be a requirement to liaise with partners and stakeholders to ensure responsive, effective, safe and person centred services are delivered.
Main duties of the job
o At all times to work in accordance with Pennine Care NHS Foundation Trust policies andprocedures o The post holder will work in partnership with all members of the Multidisciplinary Team toachieve the 'Good Health' objectives of 'Valuing People'o To provide a safe, effective, responsive, efficient, timely, community nursing service topeople with learning disabilities, their families and carers.o To undertake a range of comprehensive assessments for individuals with a learningdisability and complex and challenging needs. This assessment work requires aspecialist knowledge base on which to practice.o To monitor and evaluate complex interventions
