An exciting opportunity has arisen for an experienced HCSW to join our Community Rehabilitation Team (CRT) in the Dunfermline Locality.
The CRT provide rehabilitation to the adult population within the locality. This will be delivered to service users as out-patients or within their own home or homely setting.
The primary function of this role will be to work with the Occupational Therapy and Physiotherapy Services within CRT to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital. Occupational Therapists and Physiotherapists will provide specialist assessments to this patient group and allocate work to this post holder. This post will primarily deal with the function of non-daily rehab in the community.
