Role summary
This role is for an enthusiastic Rehabilitation Assistant to join a supportive and innovative team delivering rehabilitation in people's homes and a dedicated centre. The assistant will play a key role in facilitating early hospital discharge, preventing unnecessary admissions, and helping individuals achieve their rehabilitation goals. This involves implementing rehabilitation plans, providing direct patient care with a therapy focus, and supporting daily living skills. Full training will be provided, and the role requires flexible working across a 7-day rota.

- Working in health/social care related post or worked in a voluntary or informal caring capacity with adults
- Ability to work with a wide range of people
- Experience of working within a community setting

- Knowledge of aids and equipment used to promote independence
- Knowledge of the importance of risk assessment
- Maths and English GCSE grade 4 (or equivalent) or the willingness to undertake the courses.
- Evidence of ongoing training and personal development.
- Care Certificate or the willingness and ability to undertake the course. (This is applicable for all non-registered support workers that provide health care).
- QCF level 3 in Health, Social, Diagnostic and Therapeutic Support, or equivalent.
- Additional training in rehabilitation or caring for the older person
- Genuine desire to work with older people.

Job summary
We are a supportive friendly and innovative rehabilitation team delivering rehabilitation in peoples own homes and in our well equipped Rehabilitation Centre.
We are looking for an enthusiastic individual, interested in developing the skills to keep our population healthy and independent.
Rehabilitation assistants play a key role in facilitating early discharge from hospital, preventing unnecessary admissions and helping individuals reach goals that are important to them. This may include exercise programmes, equipment provision and practicing daily living skills alongside participation in a range of health promoting groupwork such as balance and confidence and pulmonary rehabilitation.
Full training will be provided to the successful post holder and we welcome informal visits to the team or any questions you may have about this post
Community visits are undertaken across the beautiful Solway Plain to the Lake District National Park
The successful candidate will be based in Solway Community Rehabilitation team but will also be expected to cover across the Keswick and Solway ICC when needed.
Shortlisted applicants are required to attend the interview in person.
