Role summary
This role involves providing specialist nursing support to people with Learning Disabilities (LD) and mental health challenges in the community, particularly those at risk of hospital admission or placement breakdown. The post holder will conduct assessments, develop care plans, and manage a small caseload of clients requiring Section 117 aftercare plan reviews. Working within a multidisciplinary team, the nurse will collaborate with clients, support networks, and other professionals to ensure high standards of evidence-based care. The position requires independent working and effective time management across three boroughs.
Essential criteria
- Registered NMC RNLD and/or RMN
- Evidence of post-registration qualification

- Previous Experience
- Extensive post registration, experience with experience of working with a learning disability service
- Experience of working with clients with mental health difficulties
- Experience of multi-professional collaboration, partnership working with other statutory and voluntary agencies
- Experience of working in a multi-cultural environment and knowledge of methods to develop culturally sensitive practice
- Experience of managing service users who exhibit challenging and high risk behaviours, that require advanced interventions to enable effective de-escalation
- Experience of using creative and sophisticated interventions with service users in distress
- Participation in audit cycle
- Project Management
- Knowledge/understanding of the physical and mental needs of service users being cared for within this service
- Knowledge of risk assessments and crisis and contingency planning
- Knowledge of clinical supervision process and the ability to train others
- Understanding of research and audit methodology.

- Able to facilitate multidisciplinary debriefing, critical analysis meetings
- Able to negotiate constructively and effectively with all professionals at all levels
- Able to analyse and solve problems in a professional nursing context

Community Nurse (Mental Health and Learning Disabilities)
Tri-borough - Hammersmith and Fulham, Kensington & Chelsea and Westminster.
The job role includes the following:
- To provide specialist nursing support to people with a Learning Disability (LD) at risk of breakdown or admission to mental health hospital for assessment and treatment on the Dynamic Support Register.
- To provide specialist nursing support to clients at risk of placement breakdown due to mental health difficulties.
- As part of a team to review Section 117 aftercare plans for clients known to the ICB residing within the three boroughs.
We are looking for:
- a clinician with experience of, or a strong interest in, working with people with learning disabilities and mental health challenges
- a clinician who keeps the client at the centre of their practice.
- a dedicated motivated individual who has the experience to work independently within a multidisciplinary team
