Role summary
This role is for a Mental Health Nurse, Social Worker, or Occupational Therapist team leader to join a primary care Mental Health Team in West PCN. You will be based in GP surgeries, working with adults and older persons with mild to moderate mental health difficulties. The role involves contributing to triage, offering early assessment, brief interventions, and signposting. You will also help develop systems and processes for care delivery.
Key responsibilities
- Contribute to the triage service.
- Offer early assessment, brief interventions, and signposting for people with mental health needs.
- Play an active part in the development of systems and processes which support care delivery.
- Build positive partnerships with key stakeholders in primary and secondary care.
- Work collaboratively with Solent Mind and peer recovery workers.
Essential criteria
- Registered Mental Health Nurse, Social Worker or Occupational Therapist
- Computer skills
- 2 years post qualifying experience
- Mentorship course
- Clinical supervision training

You will work within scope of practice so if have worked within specialities these skills will be transferable
