Role summary
This role involves acting as a key worker for care home service users, developing and reviewing their care plans to promote quality of life and independence. You will support re-enablement for both short-term and long-term residents. Responsibilities include acting as a shift leader, making decisions about service user welfare and safety, and assessing progress against care plans.

Previous experience in a social care setting is essential and ideally you will hold an SVQ 3 in Social care or as a minimum hold SVQ 2 with a willingness to achieve an SVQ 3 in Social Care to enable registration with the SSSC as a Practitioner in a Care Home Service for Adults.
