Role summary
This role is for a Band 6 District Nurse or Senior Community Nurse to join a community nursing team in Warwick & Kenilworth. The successful candidate will act as a proactive clinical leader, driving high-quality, community-based care with a focus on public health, prevention, and managing complex conditions. Responsibilities include leading population health assessments, care planning, staff development, and service improvement initiatives. Key responsibilities
- Manage a defined caseload, ensuring professional accountability and delegation of tasks.
- Provide visible leadership for an equitable, evidence-based service underpinned by clinical governance.
- Prescribe within individual scope of practice (V300 qualification) safely and cost-effectively.
- Develop caseload profiles, identify specific health needs, and implement care programmes using a case management approach.
- Clinically supervise and support regulated and unregulated team members, fostering career development and clinical capabilities.
- Participate in recruitment, retention, wellbeing strategies, quality improvement, and budget management.
Essential criteria
- Registered Nurse (1st Level)
- First level degree in related subject
- Evidence of Continuous professional Development relevant to the field of practice
- Evidence of leadership or management training
- Practice Educator and Assessor Preparation (PEAP) or equivalent qualification
- Significant Post registration community experience
- Post registration experience of caseload management
- Evidence of Personalised Care planning and complex patient management
- Ability to take appropriate action to assess and manage risk
- Experience of empowering a positive learning culture for interdisciplinary and interagency teams
- Proven experience of working autonomously in challenging situations
- Understanding of clinical governance, audit, and risk management.
- Advanced clinical decision-making skills to support patient centred care
- Advanced clinical skills to include a variety of patient conditions and comorbidities
- In-depth understanding of community nursing and long-term condition management.
- Must have excellent prioritisation and time keeping skills
- IT Literate
- Ability to supervise, support, and mentor junior staff
- Ability to work collaboratively across professional and organisational boundaries
- Communicate effectively using a range of skills and strategies with colleagues and people at all stages of life and with a range of mental, physical, cognitive and behavioural health challenges
- Practise autonomously, proactively and innovatively, demonstrating selfawareness, emotional intelligence and openness
- Willingness to work flexibly and travel across the district.
- Excellent role model demonstrating compassion and a patient-centred approach

- Must have means of own transport and able to drive in variable weather conditions
