Role summary
This role involves providing expert interventions and rehabilitation within the Rapid Response & Intermediate Care (RRIC) service in Buckinghamshire. The primary goal is to help patients remain in their homes, support recovery, prevent hospitalisation, and facilitate early hospital discharge. You will work as part of a multi-disciplinary community team, conducting specialist assessments, planning care, and managing patient journeys. The position requires collaboration with various professionals to ensure timely and appropriate care delivery.
Key responsibilities
- Provide triage, comprehensive specialist assessment, review, case management and evaluation of the needs of patients and carers.
- Be responsible for planning programmes of care to promote health gain and maximise independence, actively case managing patients through their journeys.
- Allocate appropriate workloads to members of the Rapid Response & Intermediate Care (RRIC) team.
- Work with a range of professionals in order to ensure care is appropriate and delivered in a timely manner.
- Demonstrate professional skills and competence in leadership, care planning / co-ordination, and care and treatment delivery.
- Support the development of clinical pathways.
Essential criteria
- Registered Allied Healthcare Professional with HCPC - Occupational Therapist
- Degree/ Diploma in Occupational Therapy
- Experience of recent Continuous Professional Development.
- Relevant post graduate clinical training e.g. Elderly Long Term Conditions Respiratory Disease Cardiac Disease Trauma, Orthopaedics & End of Life
- Demonstrate experience post or pre-registration placements in relevant clinical areas for therapists:- Care of Elderly Orthopaedics Neurology Respiratory Palliative Care Long term conditions
- In depth experience of working within multi-agency/multi-disciplinary team
- Demonstrate profession specific clinical assessment planning delivery and evaluation skills.
- Ability to motivate staff.
- Excellent written and verbal communication skills.
- Self-Awareness
- Presentation skills
- Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire.
Desirable criteria
- Experience participating in research or audit project.
- Experience of working in a community/in-patient setting for a minimum of 2 years post registration
- Experience of working with single assessment process.
- Experience of electronic clinical systems.
- Experience of day-to-day staff management e.g. case allocation to Health Care Assistants
- Ability to manage complex cases.
- Teaching patients, carers, and colleagues
- Presentation skills
- IT Skills.
- Report writing skills.
- Knowledge of modernisation agenda within the NHS and social care.
- Ability to develop and maintain partnership working.
- Knowledge of legal requirement for working with vulnerable adults and those who lack capacity.
