Role summary
This role is for a motivated and compassionate practitioner to join Leeds Community Healthcare NHS Trust's new Proactive Support Service. Key responsibilities
- Provide assessment, consultation, direct intervention, and system-wide support for children and young people.
- Work closely with families, carers, residential providers, social care, education, and health colleagues.
- Support and empower networks around the child to build resilience and reduce crisis.
- Help others understand and respond to behaviours of distress through trauma-informed and strengths-based approaches.
- Work intensively with young people at risk of placement breakdown, care entry, or delayed discharge.

Job summary
We are seeking a motivated, compassionate and values-driven practitioner to join Leeds Community Healthcare NHS Trust's new Proactive Support Service.
We welcome applications from Registered Nurses, particularly those with experience in Learning Disabilities, Mental Health, Children's Services or Complex Care, as well as Social Workers and other suitably qualified practitioners with the skills, experience and values to support children and young people with complex needs.
Funded by Leeds Integrated Care Board and delivered in partnership with Leeds City Council's Corporate Parenting Team, this innovative multidisciplinary service supports children and young people at risk of care entry, placement breakdown, or delayed discharge from hospital.
The service promotes stability, prevents escalation into care and improves long-term outcomes for vulnerable young people.
The team also supports the Children with No Onward Destination (CNOD) pathway, working with young people who are medically fit for discharge but unable to leave hospital due to placement instability, accommodation issues or complex social circumstances.
