Role summary
This is a part-time, fixed-term Static Community Physiotherapist role within Sheffield's Integrated Care Team. The post involves providing physiotherapy assessments and treatment programmes to patients in their own homes, including residential care settings, to facilitate early hospital discharge and prevent admissions. The successful candidate will work within a multi-disciplinary team, contribute to the training of therapy assistants, and manage equipment provision and referrals. The service operates 7 days a week, requiring flexible working.
Key responsibilities
- Carry out Physiotherapy and generic therapy assessments for patients in their own homes.
- Plan and deliver treatment programmes for patients in their own homes, including residential care settings.
- Work closely with therapy assistants, contributing to their training and development.
- Facilitate therapy assistants to provide care and rehabilitation to a designated caseload.
- Order, transport, provide, and fit equipment for patients.
- Make onward referrals to other health and social care services.
Essential criteria
- BSc/Diploma in Physiotherapy
- Current HCPC registration
- Student fieldwork experience in core areas of Musculoskeletal, respiratory and neurology
- Evidence of continuous personal & professional development
- Good basic knowledge of therapy practice for medicine, orthopaedics, neurology & respiratory
- Competency in Physiotherapy clinical reasoning, processes and practices
- Good interpersonal, communication and team working skills
- Able to maintain confidentiality
- Able to act on own initiative, organise and prioritise work and manage multiple tasks
- Able to maintain accurate written records & statistics
- Calm approach to changing work situations
- Able to teach and motivate others
- Motivated to learn
- Flexible and reliable approach to working practices
- Car Driver
- Full UK Driving licence

Job summary
An exciting opportunity has arisen to be part of Sheffield's community based Integrated Care Team (ICTT) in the North of the city.
This post is fixed term for 9 months and is part time 22 hours.
This is a city-wide service, operating 8 am - 6 pm, 7 days a week (including Bank Holidays). The successful candidate will be required to work flexibly to achieve this. The service works to facilitate early discharge of patients from hospital and supports patients at home to prevent unnecessary hospital admission, together with providing community-based therapy for patients with a variety of conditions including neurological, orthopaedic, frailty, musculoskeletal and anxiety issues.
This post will be based within a well-established, multi-disciplinary team consisting of: Physiotherapists, Occupational Therapists, Specialist/Senior Therapy Assistants and Administrative staff, with access to mental health nurses and co-located with the Integrated Care Team Nursing.
