Role summary
This role is for a Specialist Public Health Nurse focused on improving the health and wellbeing of Children and Young People in Care. The successful candidate will undertake health assessments, provide care recommendations, and offer health promotion advice to children, young people, and their carers. It involves managing a caseload, participating in team training, and collaborating with multi-agency partners to ensure positive outcomes for vulnerable children. The position is part-time, 24 hours a week, and based at Erlegh House in Reading.
Key responsibilities
- Undertake health assessments for children and young people in care aged 0-17 years.
- Assess and provide recommendations on plans of care for children and young people in care.
- Provide health promotion advice to children and young people in care and their carers.
- Be responsible for a caseload as delegated by the Specialist Nurse.
- Participate in team training regarding the health needs of children and young people in care.
- Contribute to inter-agency assessments and risk analysis.
Essential criteria
- Registered Nurse
- NMC Registration
- Public Health qualification (e.g. SCPHN) or willingness to undertake and specialist knowledge acquired through post graduate short courses, training and equivalent experience, in a relevant field.
- Evidence of continuing professional development
- Demonstrable experience of working autonomously to deliver high quality and evidence-based nursing care.
- Experience in providing supervision to junior colleagues and students in the completion of routine nursing and clinical skills.
- Uphold the Trust's values of being caring, committed and working as team through demonstratable examples and experience.
- Proficient use of digital tools and applications including Word, Outlook and patient management systems.
- Ability to travel between multiple Trust sites and non-Trust locations, including patient homes, some of which may not be easily accessible via public transport.
