Role summary
This role involves providing non-clinical support to the Colorectal cancer specialist nursing teams, focusing on delivering personalised care to patients affected by colorectal cancer. Key responsibilities
- Support Multi Disciplinary Team meetings and outcomes to ensure timely management of patients on a cancer pathway.
- Deliver personalised care through Holistic Needs Assessments, health & wellbeing offers, and referrals to support services.
- Liaise with community and primary care teams to facilitate timely management and treatment for patients.
- Advocate for patients, helping them navigate health & social care and empowering them to make informed choices.
- Provide support to patients who may be upset or distressed, considering health literacy and inequalities.
- Deal with enquiries from people dealing with cancer via face-to-face, phone, or email interactions.

Job summary
We have an exciting opportunity to recruit 2 band 4 Cancer
Care Coordinators to work alongside our Colorectal cancer specialist nursing
teams providing non-clinical support. These exciting role will underpin the
delivery of the personalised care agenda to people affected by colorectal cancer.
One post will be based in Darlington Hospital and one in
Durham Hospital.
We are looking for highly motivated individuals who have
excellent communication skills, are empathathetic, excellent problem solvers,
and have a passion for delivering high quality person-centred care. You will provide
the nursing team with administrative support, alongside non-clinical support
and signposting for patients.
You will be qualified to at least NVQ Level 3 or have
equivalent experience, possess good IT skills and have experience working in a
healthcare setting. You will also be able to demonstrate resilience, compassion
and a willingness to embrace innovation.
