Role summary
This Assistant Practitioner role supports the Health Visiting Service in Caerphilly, undertaking delegated tasks from a Registered Practitioner. The post holder will deliver preventative services to children and young people, contributing to their health and wellbeing outcomes. This involves delivering care packages for families and groups in the community, following the Health Child Wales Programme and other relevant acts. The team is enthusiastic and motivated, offering support to new members.

Job summary
Assistant Practitioner, Band 4, working for the Health Visiting Service, undertaking delegated tasks by a Registered Practitioner / SCPHN HV.
The ABUHB, Caerphilly Health Visiting Team are enthusiastic, motivated and forward thinking in delivering care to children and their families. You will be supported in your role by other professionals to become a valued member of the team.
Main duties of the job
An exciting opportunity has arisen for an Assistant Practitioner to work with the Health Visiting team in Caerphilly borough , delivering services to Children and Young People providing a preventative service as identified in the needs assessment carried out by the Registered Practitioner. The aim is to achieve equity of health and wellbeing outcomes following the Health Child Wales Programme (HCWP), The Social Services and Wellbeing Act and Childcare, Play and Early Years Workforce plan. The post holder will contribute to the overall healthcare/social care offered to individuals, families and communities, including delivering care packages for families and groups in the community.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
