Key responsibilities

- Undertake comprehensive mental health assessments and develop, implement, and review care plans
- Deliver evidence-based interventions for individuals with complex and co-existing needs
- Safely administer, monitor, and review medication in line with NMC and Trust standards
- Assess and manage clinical risk, escalating concerns appropriately and contributing to risk management processes
- Work collaboratively with GPs, primary care, and partner agencies to support safe discharge and continuity of care
- Maintain accurate, timely clinical records to support care delivery and audit requirements
