Role summary
This role involves delivering evidence-based, holistic mental health care within an integrated multi-disciplinary community mental health team. The post holder will manage a clinical caseload, provide person-centered assessment, diagnosis, treatment, and evaluation of care. They will work collaboratively with general practice teams and support people living in their communities. The position requires a highly motivated individual with excellent clinical knowledge and communication skills.
Key responsibilities
- To take responsibility for a clinical caseload in accordance with the Mental Health Measures.
- Foster robust working relationships with all partners.
- Contribute to the continuing improvement and development of mental health services.
- Provide support and intervention through holistic person-centered assessment, clinical consideration, diagnosis, treatment and evaluation of care.
- Demonstrate safe, clinical decision-making and expert care and support for people within their homes.
- Manage a caseload of patients with mental health issues, subject to CTP.
Essential criteria
- Experience of either inpatients or community mental health services
- Qualified NMC registrant - RMN
Desirable criteria
- Previous experience in the community setting
- Previous experience of caseload management
- Venepuncture trained
- Therapies qualification
- ECG Trained
- Welsh Skills Desirable (Level 3/B2 or above)

Job summary
To deliver evidence based holistic professional mental health care to people within the context of an integrated multi-disciplinary, multi-agency integrated community mental health team.
The post holder is identified as a skilled and creative practitioner with knowledge of working with and supporting people living within their own communities and will be an integral member of the Community Mental Health Team.
Acting within their professional boundaries, the post holder will provide support and intervention for the presenting person through holistic person centered assessment, clinical consideration, diagnosis, treatment and evaluation of their care within a multi-agency team approach. They will demonstrate safe, clinical decision-making and expert care and support for people within their homes.
The post holder will work collaboratively with the wider multi-disciplinary general practice teams to meet the needs of people within their community.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
