Role summary
This role is for a highly organised Planner and Administrator to join the new Home First service, which helps people regain independence after hospital discharge. You will coordinate patient visits, manage service capacity, and provide comprehensive administrative support to the multidisciplinary team. The position involves working a 1 in 5 weekend rota and is based at the Home First Hub in Brockworth. Essential criteria
- Educated to GCSE level in Mathematics and English to Grade C and above.
- NVQ level 3 in administration or an appropriate equivalent qualification or experience.
- RSA Typing Level 2 or equivalent qualification or experience.
- LENGTH AND / OR NATURE OF EXPERIENCE
- Experience of unsupervised administrative work in a busy office environment
- Experience of working in a customer-facing environment with evidence of telephone communication skills
- Experience supporting teams or managers and setting up/running effective office systems
Desirable criteria
- Previous experience of working with the NHS/local authority
- PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE
- Skilled in Microsoft Office Packages, e.g. Word, Excel, and PowerPoint and use of computers and keyboards
- Confident to liaise with other organisations and people as appropriate.
- Previous experience and competency of an electronic patient records (e.g. SystmOne)

The Home First service is transforming the way support is delivered to people following discharge from hospital, helping them regain independence and remain safely in their own homes. We are seeking a highly organised and proactive Planner and Administrator to play a vital role in the success of this new countywide service.
Working at the heart of the operational team, you will be responsible for coordinating patient visits, managing service capacity and demand, and ensuring resources are deployed effectively to meet the needs of the people we support. You will also provide comprehensive administrative support to the multidisciplinary team, acting as a key point of contact for staff, patients, carers and partner organisations.
