Role summary
This is a fixed-term, part-time Band 4 Care Act Assessor role within the Initial Response Team in Lichfield, lasting 9 months. It offers an opportunity for individuals starting or exploring a career in adult social care, providing exposure to various experiences and support from experienced professionals. The role involves assessing needs, signposting, information provision, care planning, and reviewing in line with The Care Act (2014) and Mental Capacity Act (2005), working closely with service-users, carers, and partner agencies. A good knowledge of community services is beneficial but not essential, as full training will be provided.
Key responsibilities
- Work alongside experienced social workers, social care assessors and senior Care Act assessors.
- Contribute towards ensuring safe, supportive and appropriate care which will support independence.
- Provide assessment of need, signposting, information, care planning and reviewing in line with The Care Act (2014) and Mental Capacity Act (2005).
- Work closely with service-users, carers, partner agencies and other professionals.
- Take responsibility for your own Continuous Professional Development.
Essential criteria
- Level 3 Diploma in Adult Care (Regulated Qualifications Framework) or equivalent, plus additional knowledge and experience of care practices and procedures to diploma level.
- A clear understanding of and commitment to equal opportunities and anti-discriminatory practice
- Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
- An ongoing commitment to personal and professional development
- The ability to demonstrate a commitment to working in partnership with adults, carers, professionals and voluntary agencies, to provide a comprehensive service which meets the needs of the adults in line with current legislation and directorate, local authority & Trust policies
- Experience of working in a team, incorporating good interpersonal relationships and the ability to actively contribute to the implementation, development and review of an individual's needs
- Experience of working within the specialist area
