Role summary
This role is for a motivated Healthcare Support Worker to join a multidisciplinary team, providing care to patients in their own homes and residential settings. You will conduct independent visits, deliver high-quality, person-centred care, and work closely with nurses and therapists. The position involves monitoring and reporting changes in patient conditions and requires travel across the Droitwich, Ombersley and Rural patch. Shifts are part of a 7-day service, including weekend and bank holiday working.
Key responsibilities
- Carry out independent visits to patients in the community.
- Deliver high-quality, person-centred care.
- Work closely with nurses, therapists and other members of the multidisciplinary team.
- Monitor and report changes in patients' conditions.
- Assist with personal care, monitoring and recording observations, wound care, catheter care, simple dressings, and other delegated clinical tasks.
- Maintain accurate clinical records, and contribute to care planning.
Essential criteria
- I understand that the Trust will not routinely provide sponsorship to work for this role, and employment on a time limited visa doesnt guarantee sponsorship at point of expiry.
- Level 3 qualification (e.g. NVQ 3, A-level) in Care or equivalent qualification or experience or Level 2 qualification (e.g. NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post
- Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
- Basic knowledge of physical/ mental health conditions
- A level of understanding of rehabilitation
- Ability to communicate clearly and effectively in English in both verbal and written formats
- Ability to use IT confidently and competently in daily work
- Good numeracy skills
- Aptitude for promoting independence
- Ability to cope with difficult/emotionally distressing/ challenging situations
- Able to work autonomously and as part of a team
- Good organisational and time management skills
- Good observational skills
- Ability to work without direct supervision and able to use own initiative
- Experience of working with older adults in a health/social care setting
Desirable criteria
- Community experience of older adults with physical and/or mental health problems
- Experience of working in a rehabilitation or general medical setting
- Experience of working and liaising with staff from different disciplines
- Experience of medication administration
