Role summary

This role is for a Band 6 Mental Health Practitioner within the Primary Care Mental Health Practitioner Team, part of the Neighbourhood Mental Health Team. The post involves providing rapid and responsive assessment, signposting, and brief treatments for patients with mild to moderate mental health needs. Key responsibilities

  • Deliver rapid and responsive triage, assessment, signposting, and brief treatment (1:1, group based, and digital platforms) for patients with mild to moderate mental health needs.
  • Embed mental health expertise in a primary care setting, working with VCS and NMHT colleagues.
  • Provide advice and consultation to VCS colleagues on managing complexity and risk.
  • Conduct clinical assessment, signposting, and brief treatment to individuals referred by VCS colleagues.

Essential criteria

  • Registered Mental Health qualification or equivalent
  • Include relevant professional registration
  • Extensive post-qualifying experience of clinical work with people with a range of mental health problems
  • Experience of partnership working within and across various different statutory and non-statutory teams and agencies
  • Experience in working consultatively with professionals
  • Experience of providing low intensity psychological interventions including, provision of self help, behavioural activation, solution focused, guided self help, and a CBT informed approach using a range of delivery models, telephone, face to face , individual and workshops
  • Experienced in conducting clinical assessments, including risk assessments

As we work towards our goal of providing an integrated service across primary and secondarycare, our community mental health services are being transformed to recognise the changes inhow people want to access them. We need to support those who have experienced barriers toaccessing services which support their mental health, due to, the complexity of their needs, or theconfiguration of our services.

We are seeking practitioners with experience of providing rapid and responsive assessment,signposting and brief treatments to join us at this exciting time as a Band 6 Mental HealthPractitioner (MHP). You will be a qualified Nurse or Allied Health Professional, with the relevantprofessional registration (NMC, HCPC).

Employed by SPFT, this Mental Health Practitioner post works within the Primary Care Mental Health Practitioner Team within the Neighbourhood Mental Health Team (NMHT). The role will support the embedding of specialist mental health expertise at a neighbourhood level within the Emotional Wellbeing Services.
