Role summary
This role is for a Band 5 NMC Registered Staff Nurse to join community nursing teams in North Staffordshire. You will provide holistic care to patients in their own homes, including palliative care, long-term condition management, and wound care. The role involves working autonomously within a multidisciplinary team and requires travel across the area. It includes daytime shifts, weekends, and early mornings.
Essential criteria
- Degree in nursing or equivalent
- Practice Supervisor/ Assessor qualification or willingness to work towards
- Current Active NMC Registration
- Ability to manage time & caseload effectively
- Relevant clinical experience to the role e.g. assessment skills & wound care
- Supporting service users, families and carers in dealing with people with nursing needs
- Mutidisciplinary team working
- Record keeping

- Biddulph & Kidsgrove PCN based at Kidsgrove Health Centre
- Hanley Bucknall & Bentilee PCN based at Bentilee Neighbourhood Centre
- Newcastle South PCN based at Bradwell Hospital
