Role summary

This Band 6 Care Co-ordinator role is for a passionate nurse to support patients receiving clozapine and intramuscular (depot) medication in the community. The successful applicant will have overall responsibility for providing this essential clinic to patients within the London Borough of Merton. The role involves physical health monitoring, managing blood results, and ensuring continuity of care. It also offers opportunities for professional development and teaching.

Essential criteria

  • Demonstrated experience of working with people who experience serious ongoing mental health problems
  • Experience of working in a multidisciplinary team
  • Experience of assessing and managing crisis with those with a severe mental illness.

Desirable criteria

  • Phlebotomy training
  • Previous experience of working with service users on Clozapine
  • Education and Qualification
  • RMN (current NMC registration)
  • Evidence of continuing Professional Development
  • Knowledge & Skills
  • Excellent written and verbal communication skills
  • Ability to assess clinical situations quickly and use initiative
  • Good time management skills and ability to prioritise
  • Good IT skills

Job summary

A great opportunity exists with a vacancy arising for the role of Care Coordinator in The Richmond Integrated Recovery Hub the We are open to applications from Community Psychiatric Nurses - tailored to clozapine and Depot administration. The service offers support to adults aged 18-75 with a range of mental health conditions.

The successful applicant will have scope to develop as a Clinician, in a multi-disciplinary team. Care Coordinators reports the Team Manager who reports to the Clinical Manager for Adult Community Services in the Borough of Richmond.
