Role summary

This role involves supporting patients with safe and timely discharges from hospital to their community destination. You will provide compassionate, person-centred care during transfers and communicate effectively with patients, relatives, and colleagues. The position requires home visits and lone working, with workload allocated by a registered professional. You will also maintain patient records and liaise with other departments.

Key responsibilities

  • To support the Discharge Team in facilitating timely discharges for complex patients
  • Provide safe, compassionate and person centred care and support to patients during their transfer from hospital to their community discharge destination
  • Home visits and lone working without direct supervision
  • Transfer patients to the discharge lounge if required prior to supporting on community transport
  • Maintain excellent written patient records, demonstrating patient confidentiality in accordance with Caldicott guidelines
  • Provide support and mobility including personal care, if needed

Essential criteria

  • NVQ - Level 2 or equivalent experience in caring role
  • GCSE level of education
  • Willing to study for Care Certificate
  • Experience in patient/customer facing role
  • Able to communicate in complex situations
  • Understanding of the NHS

Desirable criteria

  • NVQ 3 or equivalent
  • 2 years experience in patient facing role
  • understanding of NHS discharge process

Clear communication between the Hospital Discharge Team, Discharge to Assess Hub, D2A, ICAH and the wards to ensure safe timely discharge of all patients .
