Role summary
This is a permanent, part-time Band 4 Care Act Assessor role within the Community Teams Adult Social Care in South Staffs. The role involves working with service-users, carers, and partner agencies to provide assessment of need, signposting, information, care planning, and reviewing in line with The Care Act (2014) and Mental Capacity Act (2005). It's an ideal opportunity for someone at the beginning of their social care career, offering full support for skill development and career progression. The successful candidate will need to travel across Staffordshire and out of county, requiring a clean driving licence and a vehicle for business use.
Key responsibilities
- Work closely with service-users, carers, partner agencies and other professionals.
- Provide assessment of need, signposting, and information.
- Conduct care planning and reviewing in line with The Care Act (2014) and Mental Capacity Act (2005).
- Take responsibility for own Continuous Professional Development.
Essential criteria
- Level 3 Diploma in Adult Care (Regulated Qualifications Framework) or equivalent, plus additional knowledge and experience of care practices and procedures to diploma level.
- A clear understanding of and commitment to equal opportunities and anti-discriminatory practice
- Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
- An ongoing commitment to personal and professional development
- The ability to demonstrate a commitment to working in partnership with adults, carers, professionals and voluntary agencies, to provide a comprehensive service which meets the needs of the adults in line with current legislation and directorate, local authority & Trust policies
- Experience of working in a team, incorporating good interpersonal relationships and the ability to actively contribute to the implementation, development and review of an individual's needs
- Experience of working within the specialist area
