Role summary
This role involves providing safe and effective medicines optimisation services to patients referred into the Integrated Pharmacy Team and community hospital sites. You will offer pharmaceutical advice to specialist services and develop expertise in optimising medicines for frailty patients. The position requires working with multidisciplinary teams to achieve the best medicine-related outcomes.
Key responsibilities
- Optimising medicines choice through medication review for patients in their own home or community hospital setting
- Ensuring safe transfer of care into community hospitals including medicines reconciliation
- Participation in KCHFT and primary care network (PCN) multidisciplinary team patient reviews
- Assessment of concordance and implementation of appropriate interventions
- Providing responses to medication queries
- Provision of legal and ethical advice as appropriate for the service provision
Essential criteria
- MSc in Pharmacy (entry level for pharmacy)
- Registration with General Pharmaceutical Council
- Post registration community or hospital experience
- Understanding of practical medicines management issues in community based services
- Commitment to mandatory continual professional development (CPD)
- Understanding of patient centred medicines optimisation including deprescribing
- Expertise on legal and ethical pharmaceutical issues in all areas relating to community health services
- Experience and knowledge of the supply, distribution, storage and administration of pharmaceuticals in Community Hospital and Primary Care settings
- Knowledge of risk management and assurance issues relating to all aspects of medicines use
- Knowledge of other professions and their registering bodies e.g. Nursing and Midwifery Council (NMC)
- Knowledge of the changing NHS environment and structures
- Understanding of Government Policy and vision for the development of prescribing and pharmacy Services
- Knowledge of storage, use and disposal of pharmaceutical products with due regard to local and national policies
- Understanding of the concepts of Clinical Governance
- A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country) and the ability to travel independently are essential for this role. As part of a team of Clinical Pharmacists and technicians you will be responsible for providing safe and effective medicines optimisation services to patients referred into the Integrated Pharmacy Team (IPT) and agreed community hospital sites. In addition, you will provide pharmaceutical advice to a range of specialist services.
