Role summary
Join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Staff Nurse. This role involves providing a safe and effective alternative to hospital admission for patients experiencing acute health changes, supported by face-to-face visits and digital technology. You will deliver agreed treatment/care plans to service users in their own homes, working within a multidisciplinary team to avoid unnecessary hospital admissions. The position is part of a seven-day service.

- NMC Registered
- Previous community experience
- Pre or post registration experience in general nursing practice
- Previous NHS experience
- Experience of working in a multi disciplinary / multi-agency health / social care environment
- Experience and knowledge of IT basic administration systems including Microsoft Office Suite (Word, Excel etc).
- Knowledge and Skills
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Ability to carry out comprehensive holistic nursing assessment
- Awareness of current community nursing skills
- Awareness of changing healthcare factors influencing change

An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Staff Nurse. The post will form part of a seven day service based within one of the Trust's locality teams. The service provides patients who are experiencing an acute change in their health, with a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.
We are looking for a Registered Nurse who shares our passion for providing excellent quality of care and can help deliver a recommended and agreed treatment/care plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.
