Role summary
The West Wiltshire Community Mental Health Team is seeking a motivated and compassionate Community Psychiatric Nurse for their Psychosis Pathway. This role involves supporting individuals with severe and enduring mental health conditions in a community setting. The team uses a strength-based, intervention-led model of care, focusing on personalized well-being and safety plans. The position offers a supportive team environment and opportunities for professional development.
Key responsibilities
- Undertake the full range of activities required to deliver an ongoing comprehensive mental health service.
- Facilitate the delivery of Personalised Care and Support Planning in line with the Your Team your Conversation-Your Plan Policy and guidance.
- Record risk incidents and collaboratively create and update Safety Assessments and Safety Plans.
- Provide personalised interventions to support people with Mental Health difficulties.
- Maintain own workload and plan the workload of others, ensuring time is prioritised effectively.
Essential criteria
- Professional Registration - Registered Mental Health Nurse
- Experience in adult community mental health
- Evidence of risk management and developing risk management strategies
- Highly developed written communication skills
- Excellent documentation skills and report writing
- To be able to deliver interventions in the community and be practically able to do so. i.e - Driver
Desirable criteria
- Experience, or placements in a wide variety of settings
- experience in medication management
- Older Adult Experience/Transferrable Skills
- Child and Young Person Experience/Transferable Skills
